As part of our application process, we kindly request a recent selfie photo and a photo of the front and back of your government-issued ID. This will help us verify your identity and make your onboarding process smoother. Please ensure your face is clearly visible in the selfie against a simple background.

Any photos that are not clear, are not of you, or are missing the required ID photos will be rejected and result in an incomplete application.

Upon approval into the program, we will require your Social Security number. This is a necessary step in our verification process to confirm your identity and eligibility for participation, and to comply with relevant regulations.

Please provide the name and contact information (name, phone number and email address) for an emergency contact person. This person should be readily available in case of an emergency during the program. This person cannot be yourself.

Americans 4 Equality Warp 10 Tech Program Code of Conduct

This Code of Conduct outlines the expected behavior for all participants in the Americans 4 Equality Warp 10 Tech Program, including students, instructors, staff, and guests. We are committed to creating a safe, inclusive, and productive learning environment for everyone. Failure to adhere to this Code of Conduct may result in disciplinary action, up to and including dismissal from the program.

I. General Behavior

• Respect for Others: Treat all peers, instructors, and staff with respect, courtesy, and professionalism at all times. Bullying, harassment (including cyberbullying), intimidation, discrimination, or any form of threatening behavior will not be tolerated. This includes, but is not limited to, verbal abuse, offensive gestures, microaggressions, and any form of unwelcome physical or sexual attention.
• Inclusivity: We celebrate diversity and expect participants to be inclusive and considerate of different backgrounds, cultures, perspectives, abilities, and learning styles. Discriminatory or disrespectful comments or actions towards any individual or group based on their identity (including race, ethnicity, religion, gender, gender identity, sexual orientation, disability, socioeconomic status, etc.) will not be tolerated.
• Collaboration: Foster an environment of cooperation and mutual support. Group activities are designed to encourage teamwork; every voice should be heard and respected. Active listening and constructive feedback are crucial components of successful collaboration.
• Professionalism: Maintain a professional demeanor at all times, both in person and in online interactions. This includes appropriate language, tone, and conduct.

II. Weapons Policy

• No Weapons: Weapons of any kind, including firearms, knives, explosives, or other potentially dangerous objects, are strictly prohibited on program premises or at program-related events.

III. Attendance Policy

• Attendance Requirement: Regular attendance is essential for successful program completion. More than three (3) unexcused absences will result in dismissal from the program. Excused absences require advance notification and documentation (e.g., doctor's note, legal documentation).
• Punctuality: Students must be punctual for each session. Late arrivals disrupt the learning environment and demonstrate a lack of respect for others' time. Consistent tardiness will be addressed.
• Absence Notification: If you are unable to attend a session, notify the head instructor or program coordinator at least 24 hours in advance, if possible. Provide a brief explanation for your absence.

IV. Academic Integrity

• Honesty in Work: Students must complete all assignments and projects independently unless collaborative work is explicitly allowed by the instructor. Plagiarism, cheating, fabrication of data, or any form of academic dishonesty is strictly prohibited and will result in serious consequences.
• Proper Attribution: When using external resources (including but not limited to books, articles, websites, and code), students must give proper credit and references to the original authors using a consistent citation style (as specified by the instructor).
• Data Integrity: All data used in assignments and projects must be accurate and honestly represented. Any falsification or manipulation of data is a serious breach of academic integrity.

V. Commitment to Learning

• Active Participation: Students are expected to engage actively in all classes, discussions, and group projects. This includes asking questions, sharing ideas, contributing constructively to discussions, and completing assigned readings and preparation.
• Personal Responsibility: Take responsibility for your own learning by staying on top of assignments, preparing for classes, seeking help when needed, and utilizing available resources.
• Growth Mindset: Embrace challenges and view failures as opportunities for growth and learning. The program is designed to push you beyond your comfort zone; a positive and resilient attitude will significantly contribute to your success.

VI. Technology and Equipment Use

• Responsible Use of Devices: Personal devices should be used responsibly during sessions. Use of devices for unrelated activities (such as social media, games, or unrelated web browsing) during class time is disruptive and disrespectful. Instructors may restrict device use at their discretion.
• Respect for Program Equipment: Any equipment or materials provided by the program should be treated with care. Damage due to misuse may result in a financial responsibility to the student.
• Software Usage: Use of program software and resources is limited to program activities. Unauthorized access or distribution of software is prohibited.

VII. Communication

• Professional Communication: Whether in person, via email, or on online platforms, students are expected to communicate in a respectful and professional manner.
• Timely Responses: When communicating with instructors or staff, aim to respond promptly, particularly when information is time-sensitive.

VIII. Safety and Conduct in Physical Spaces

• Health and Safety Compliance: Follow all health and safety guidelines provided by the program. This includes proper use of equipment, adherence to any relevant health protocols, and respecting space capacities.
• Respect for Facilities: Keep all program spaces clean and orderly. Do not bring food or drink into areas where they are prohibited.
• Emergency Procedures: Familiarize yourself with emergency procedures and evacuation routes.

IX. Confidentiality and Privacy

• Respect for Privacy: Do not share personal information, class discussions, or materials shared by others without explicit permission. Maintain confidentiality where required.
• Use of Images or Media: Students should not record or photograph any part of the program without prior approval from both the program coordinator and all participants involved.
• Media Release: By agreeing to this Code of Conduct, I consent to Americans 4 Equality using my photograph(s) taken during the Warp 10 Tech Program on their website, social media, and in publications.

X. Feedback and Improvement

• Constructive Feedback: Students are encouraged to provide constructive feedback to instructors and peers. Be specific, kind, and solution-oriented.
• Open to Feedback: Similarly, students should be open to receiving feedback from others and take it as an opportunity for self-improvement.

XI. Consequences for Misconduct

• Verbal or Written Warnings: Minor infractions may result in a verbal or written warning from program staff.
• Disciplinary Actions: Repeated or serious violations of the code of conduct may lead to removal from the program. Depending on the severity, this could be immediate or following a probationary period.
• Dismissal from the Program: In extreme cases, behavior that significantly disrupts the learning environment or endangers others will result in immediate dismissal.

XII. Reporting Concerns

Any violation of this Code of Conduct should be reported to programs@mlkgandhi.org.

All reports will be handled confidentially to the extent possible.
